Effective April 10, 2020, and continuing through August 31, 2020, State of Emergency orders temporarily modified the enrollment requirements to support "social distancing" and "minimal contact" while continuing to support recipients receiving needed care.
The enrollment requirements have been temporarily changed to avoid group meetings. This means each prospective Provider will be able to enroll as an IHSS Provider by submitting documents by U.S. mail and/or by visiting our office and leaving the documents in our lobby drop box. Below is a brief outline of the steps to follow for the temporary enrollment process. Please look closely for the imbedded links below which will take you to additional web pages with more detailed explanations on various subjects.

Step 1: Provider completes the "Provider Enrollment" form (SOC 426).
Step 2: Recipient completes the "Recipient Designation of Provider" form (SOC 426A).
Step 3: Mail or drop off all the original documents listed in items 1-2 above to the address at the bottom
of any of our web pages.
Step 4: Provider completes the "Request for Live Scan Service" and visits a local live scan operator. Click

Additionally, we request a color photo copy of your valid ID and Social Security Card be mailed to our office at the address at the bottom of this web page.
- Provider makes a color photo copy of their valid identification card/document (front and back).
- Provider makes a color photo copy of their social security card (front and back).

Tips:
- Print neatly
